Job summary

CareMed Health Solutions seeks a Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities, collaborating with an interdisciplinary team. This role emphasizes geriatric care and close coordination with facility staff.

Responsibilities include managing acute changes, advanced care planning, and chronic care management, along with prescribing medications per Ohio law and coordinating with the medical team to ensure optimal resident outcomes.

Qualifications

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular basis to discuss changes in condition and care plans.
• Collaborate with primary care physician and care team.
