Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities in Ohio. You will manage acute changes, plan for advanced and chronic care needs, and collaborate with primary care physicians to optimize patient outcomes.

This role requires current Ohio NP licensure, board certification, and DEA licensure, with experience in geriatric or adult populations and EMR systems.

Qualifications

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.

Responsibilities

• Provide primary and preventative care for long term care patients in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances as per OH law.
• Order, review and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff regarding changes in condition and care plan.
• Collaborate with primary care physician.
