Job summary

Trinity Health PACE in the United States seeks a Nurse Practitioner to join our team, providing primary medical management in collaboration with PACE Organization physicians. You will perform assessments, develop care plans, and ensure participants receive comprehensive care in a warm, supportive environment.

The role requires 5–7 years of nursing experience with long-term care, an active NP license, prescriptive authority, BLS, and the ability to travel within the region.

Qualifications

• Master's degree in Nursing with 5–7 years of long-term care experience.
• Current, unencumbered license to practice in the state of PO.
• CDS Certification and DEA Registration required.
• Board Certification by ANCC or AANP.
• Certification as a Nurse Practitioner with prescriptive authority.
• Current BLS required.
• A minimum of one year working with the frail or elderly population or receive training from PACE on hiring.

Responsibilities

• Collaborate with PO physicians, performing assessments and implementing care plans.
• Participate in discharge planning and provide acute illness treatment.
• Conduct home visits when necessary.
• Assist in formulating clinical policies and procedures.
• Participate in provider on-call rotation.
