Job summary

Trinity Health PACE is seeking a Nurse Practitioner to provide primary medical management in collaboration with PACE Organization physicians. You will conduct assessments, develop care plans, and ensure comprehensive, patient-centered care for elderly participants in a warm, supportive environment.

Responsibilities include collaborating with PO physicians, discharge planning, home visits, and contributing to clinical policy development.

Qualifications

• Master’s degree in Nursing with 5–7 years of experience, preferably in long-term care.
• Current, unencumbered nursing license to practice in the state of PO.
• Nurse Practitioner certification with prescriptive authority.
• CDS certification and DEA registration required.
• BLS certification current.
• One year (or more) experience with frail/elderly population or completed training from the PACE organization on elderly care.

Responsibilities

• Collaborate with PO physicians to perform assessments and implement care plans.
• Participate in discharge planning and provide treatment for acute illness.
• Conduct home visits when necessary.
• Assist in formulating clinical policies and procedures.
• Participate in provider on-call rotation.
