Job summary

Pinnacle Care Providers seeks a qualified Nurse Practitioner to deliver primary and chronic care to older adults in SNFs, LTCs, and senior living communities in the greater Cleveland area. You will work with physicians, nurses, therapists, social workers, and facility staff to promote health, independence, safety, and quality of life.

Responsibilities include comprehensive assessments, management of chronic diseases, care coordination, and contributing to discharge planning.

Qualifications

• Master's or Doctoral degree from an accredited NP program.
• Current NP licensure in the practice state.
• National certification: AGPCNP, FNP, or AGACNP as applicable.

Responsibilities

• Perform comprehensive health assessments and physical exams.
• Diagnose and treat acute and chronic geriatric conditions.
• Develop and manage individualized treatment plans.
• Prescribe meds and monitor efficacy and safety.
• Coordinate care with interdisciplinary teams and facilities.
• Document findings in EMR and ensure CMS-compliance.
