CHS Recruiting invites applications for a Nurse Practitioner – Geriatric Primary Care in Akron, OH. This full-time role focuses on house calls and facility-based visits across ALFs/ILFs and private homes.

You will deliver comprehensive geriatric primary care, build care plans, and educate patients and families while coordinating with a diverse care team in a mobile, AI-assisted EHR environment.

Qualifications

• Ohio NP license required.
• DEA registration required.
• New grads with home health, LTC, ER backgrounds encouraged.
• No experience required.

Responsibilities

• Conduct health assessments to identify patient needs.
• Develop personalized care plans.
• Regularly evaluate patient conditions and responses to treatment.
• Adjust treatment plans as necessary.
• Perform minor procedures and wound care as required.
• Educate patients and families on health management and self-care.
• Coordinate care with physicians, nurses and other professionals.
• Work with mobile-based AI-enabled EHR to reduce administrative time.
