Job summary

Pinnacle Care Providers seeks a qualified Nurse Practitioner to deliver primary and chronic care to older adults in SNFs, LTCs, and senior living communities in the greater Cleveland area. You will work with physicians, nurses, therapists, social workers, and facility staff to promote health, independence, safety, and quality of life.

Responsibilities include comprehensive assessments, management of chronic diseases, care coordination, and contributing to discharge planning.

Qualifications

  • Master's or Doctoral degree from an accredited NP program.
  • Current NP licensure in the practice state.
  • National certification: AGPCNP, FNP, or AGACNP as applicable.

Job description

Our client, Pinnacle Care Providers is looking for a well qualified advanced practice nurse in the greater Cleveland area.

The Nurse Practitioner at Pinnacle care providers provides comprehensive primary and chronic care services to older adults residing in skilled nursing facilities (SNFs), long-term care facilities, assisted living communities, and independent living settings. The NP works collaboratively with physicians, nursing staff, therapists, social workers, families, and facility administrators to deliver high-quality, patient-centered care that promotes health, independence, safety, and quality of life.

Day-to-Day
  • Perform comprehensive health assessments, including physical examinations, cognitive evaluations, and functional assessments.
  • Diagnose and treat acute and chronic medical conditions commonly affecting geriatric populations.
  • Develop, implement, and manage individualized treatment plans.
  • Order, interpret, and evaluate laboratory tests, diagnostic studies, and imaging results.
  • Prescribe medications and monitor effectiveness, adverse reactions, and polypharmacy risks.
  • Conduct routine visits, annual wellness visits, admission assessments, and follow-up evaluations.
  • Respond to changes in patient condition and provide timely interventions to reduce avoidable hospitalizations.
  • Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
  • Participate in care conferences with patients, families, caregivers, and facility staff.
  • Provide palliative care and support end-of-life planning when appropriate.
  • Manage chronic diseases such as diabetes, hypertension, heart failure, COPD, dementia, Parkinson's disease, arthritis, etc.
  • Evaluate mental health concerns including depression, anxiety, behavioral disturbances, and cognitive decline.
  • Document clinical findings, treatment plans, and patient encounters in electronic medical records (EMR).
  • Ensure compliance with federal, state, and facility regulations, including CMS guidelines.
  • Educate patients, families, and staff regarding disease management, preventive care, and health promotion.
  • Participate in quality improvement initiatives and risk management activities.
Education and Preferred Qualifications
  • Master's or Doctoral degree from an accredited Nurse Practitioner program.
  • Current Nurse Practitioner licensure in the state of practice.
  • National certification as -
  • Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
  • Family Nurse Practitioner (FNP)
  • Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) (as applicable)
  • DEA registration and prescriptive authority as required by state law.
  • Current BLS certification.
  • Valid driver's license and reliable transportation if travel between facilities is required.
  • 1-3 years of experience in geriatrics, long-term care, skilled nursing, primary care, or post-acute care.
  • New grads with extended training and onboarding are welcome to apply
  • Experience working in nursing homes, assisted living, or rehabilitation facilities preferred.
  • Knowledge of CMS regulations, MDS processes, and quality measures.
  • Experience with EMR systems and telehealth platforms.
Physical Requirments
  • Ability to stand, walk, bend, and move throughout healthcare facilities.
  • Ability to perform physical assessments and clinical procedures.
  • Ability to lift and move up to 25 pounds occasionally.
  • Frequent use of computers, tablets, and EMR systems.

Location: Maple Heights or Cleveland, Ohio - the position will require local travel
