Job summary

Miami Jewish Health seeks a Nurse Practitioner for the FLORIDA PACE Centers to provide direct primary medical care and counseling to program participants. You will perform history and physicals, manage acute and chronic illnesses, and coordinate care with the medical director and specialists in a team-based setting.

The role requires ARNP/PA licensure, BLS, and DEA license; ACLS preferred. Experience with frail elderly populations is valued, with opportunities to engage in interdisciplinary team

Qualifications

• Graduate from an accredited nursing program with ARNP or PA licensure.
• BLS certification required; ACLS preferred; DEA license required.

Responsibilities

• Perform comprehensive history and physicals for new referrals and quarterly intervals.
• Manage patients with acute, episodic, and chronic illnesses.
• Admit patients to hospital and coordinate care with Medical Director and specialists.
• Provide care in nursing home settings and communicate with interdisciplinary teams.
• Participate in family conferences, committees, and departmental meetings.
• Document treatments in patient charts per department guidelines.
