Job summary

Senior Community Care of Michigan - PACE seeks a Full-Time Nurse Practitioner to deliver primary care to program participants under the Medical Director. The NP conducts physical assessments of new enrollees and semi-annual reassessments, and develops care plans with the Interdisciplinary Team.

This onsite role requires Michigan RN licensure, NP certification, and experience with frail elderly patients. Travel to participants' homes is occasional; on-call after hours is required.

Qualifications

RN licensed in Michigan and eligible to practice as an NP.
Graduate of a professional nursing program and NP certification.
Minimum 1 year experience with frail elderly population.
Training or related experience in the job assigned.

Responsibilities

Provide primary care to participants under Medical Director supervision.
Perform physical assessments of new enrollees and semi-annual reassessments.
Develop and implement care plans with the Interdisciplinary Team.
Evaluate physical complaints and provide appropriate treatment.
Educate participants and caregivers; coordinate with PCPs and Medical Directors.
Participate in discharge planning and on-call activities after hours.
