Job summary

Miami Jewish Health, a leading provider of healthcare and living options for aging adults in Florida, seeks a Nurse Practitioner for the Florida PACE Centers. You will deliver direct primary medical care, counsel participants, and collaborate with the medical director to support frail elders.

Responsibilities include conducting histories and exams, managing acute and chronic conditions, and coordinating care with a multidisciplinary team.

Qualifications

• Graduate from an accredited school of nursing.
• Advanced training and certification as an Advanced Registered Nurse Practitioner (ARNP) or advanced training and licensure as a Physician Assistant (PA).
• Current ARNP/PA license by the Florida State Board of Nursing; BLS certification; ACLS preferred.

Responsibilities

• Performs comprehensive history and physical on new referrals; interval history and physical quarterly or as indicated.
• Manages patients with acute, episodic, and chronic illnesses.
• Admits patients to hospital and coordinates care with medical director and specialists.
• Coordinates discharge planning with the care team and participates in family conferences.
• Documents treatment in patient charts according to department guidelines.
• Maintains professional communication with residents, families, and staff.
