Job summary

Care Guide Partners PACE (CGPP) is seeking a Nurse Practitioner to assess and coordinate direct care for program participants. The role centers on clinic/day-center encounters with possible home visits, working with the IDT and other providers to ensure comprehensive care.

Requires KY APRN licensure (DEA), BLS, and at least 1 year of advanced nursing practice with frail or elderly populations. Travel within the service area and EMR proficiency are expected.

Qualifications

• Licensure for advanced nursing practice in KY (including DEA and controlled substances) is required.
• Minimum of one year of experience in advanced nursing practice.
• Knowledge of physical, mental, and social needs of frail older adults.
• Minimum 1 year of experience working with a frail or elderly population or training upon hire.
• Must maintain Basic Life Support (BLS) certification.
• Flexibility to travel within the service area and provide care in participant homes occasionally.
• Proficiency with electronic medical record (EMR) systems.
• Ability to work independently and as part of a team with high autonomy in clinical and management decisions.

Responsibilities

• Assess, initiate, provide and coordinate direct care for CGPP participants.
• See participants primarily in clinic/day center; may visit homes, long-term care facilities, or hospitals.
• Coordinate care with the Interdisciplinary Team (IDT) and other medical providers.
• Participate in education, outreach, and leadership activities for CGPP communities.
• May be assigned additional patient care or administrative duties by Medical Director or Executive Director.
