Job summary

St. Elizabeth Physicians is seeking an experienced Geriatric Nurse Practitioner to join the PACE program. The APP will oversee care for PACE enrollees, conduct CMS visits, and collaborate with an interdisciplinary team to ensure optimal functioning and safe, coordinated care across eight Kentucky counties.

Responsibilities include coordinating with physicians, families, and community partners, and developing protocols for interventions based on current literature. KY license required or eligible.

Qualifications

• Experience in geriatrics and advanced practice nursing.
• Ability to assess complex patient needs and coordinate care.
• Strong collaboration with interdisciplinary teams and physicians.

Responsibilities

• Oversee care for PACE enrollees across eight counties in Northern Kentucky.
• Complete CMS regulatory visits and provide clinical information to the team.
• Coordinate and oversee patient care with physicians, families, and community partners.
• Develop and review treatment protocols in collaboration with the physician.
