Job summary

Mountain View PACE in Kentucky is seeking a Nurse Practitioner to assess, manage, and coordinate care for chronically ill geriatric participants. You will work with physicians, leadership, and the clinical team to deliver safe, high-quality, outcome-driven care in a supportive interdisciplinary environment.

This full-time, first-shift position offers a starting salary of $120,000 with a $3,000 sign-on and potential to earn more through the Triad Incentive Plan.

Qualifications

• Must be licensed as a Certified Nurse Practitioner in the state of practice.
• One year of experience working with the frail and elderly population or willingness to participate in extensive training as needed.

Responsibilities

• Practice within defined scope and regulatory guidelines.
• Perform health histories, physical exams, patient education, and follow-up on diagnostic results.
• Identify urgent or emergent medical conditions and initiate appropriate interventions.
• Collaborate with physicians and interdisciplinary team members to support optimal participant outcomes.
• Support clinical operations, including documentation, scheduling, and charting.
• Participate in quality improvement initiatives and care planning activities.
• Serve as a clinical resource for staff, participants, and families.
• Contribute to 24-hour care coordination.
• Attend meetings and complete other duties as assigned.
