Job summary

Elderhaus PACE seeks a compassionate Nurse Practitioner to provide primary care to frail older adults within a patient-centered, interdisciplinary PACE program in Wilmington, NC. You will assess, manage, and coordinate care to maintain independence and quality of life, collaborating with a team of healthcare professionals and caregivers.

You will perform comprehensive exams, develop treatment plans, and educate participants and families while ensuring accurate documentation and adherence to

Qualifications

• Current and unrestricted North Carolina Registered Nurse license.
• Certification as a Nurse Practitioner.
• Current CPR certification.
• Valid driver's license, reliable transportation.
• At least one year of experience with frail older adults or geriatric population.

Responsibilities

• Conduct comprehensive health assessments and physical examinations for new participants.
• Perform scheduled reassessments and evaluate health status.
• Develop, implement, and monitor individualized treatment plans.
• Evaluate and treat acute and chronic health conditions.
• Order, interpret, and follow up on diagnostic tests and labs.
• Prescribe and manage medications per licensure and protocols.
• Coordinate care with specialists, hospitals, and caregivers.
• Provide education on disease management, medications, and prevention.
• Maintain timely clinical documentation and coding.
• Participate in care planning and quality improvement meetings.
• Support staff education and uphold high standards of care.
