Job summary
Trinity Health PACE is seeking a Nurse Practitioner to join our team in Fayetteville, NC. You will conduct assessments, develop care plans, and ensure comprehensive care for participants in collaboration with PO physicians.
The role includes home visits, discharge planning, and contributing to clinical policy development, with on-call rotation as part of the responsibilities. A master's in nursing with 5–7 years' LTC experience and current NP license with prescriptive authority are required.
Qualifications
- Master's degree in Nursing and 5–7 years of experience, preferably in long-term care.
- Nurse Practitioner certification with prescriptive authority and current BLS.
- Active nursing license to practice in the state; CDS and DEA registrations.
- Board certification by ANCC or AANP.
Responsibilities
- Collaborate with PO physicians to perform assessments and implement care plans.
- Participate in discharge planning and provide care for acute illnesses.
- Conduct home visits when needed and support clinical policy development.
- Join on-call rotation and assist in policy/procedure formulation.
