Job summary

Carolina Senior Care in Lexington, NC is hiring a Nurse Practitioner or Physician Assistant to join our PACE team. Under the Medical Director, you will provide primary care, perform assessments, develop care plans, treat participant concerns, and educate participants and caregivers.

You will collaborate with the Interdisciplinary Team to deliver geriatric care, manage acute and long-term conditions, coordinate care in nursing homes and discharge planning, and maintain accurate records.

Qualifications

• Bachelor’s Degree required.
• MD/DO, PA or NP credentials with relevant board certification preferred.
• Current NC physician license or PA license with DEA registration.
• CPR certification required.

Responsibilities

• Team collaboration with the IDT to plan and adjust care plans.
• Perform assessments, exams, and develop care plans for participants.
• Manage acute and long-term care, including discharge planning.
• Maintain accurate records, coding, and ordering lab tests.
