Job summary

Career Land Center, LLC is opening a PACE center in the Stockton, CA area and is seeking a Nurse Practitioner to join its interdisciplinary care team.

The NP will provide primary care to a panel of frail, elderly participants with complex chronic conditions, collaborating with physicians and other health professionals to develop individualized treatment plans and coordinate care across settings, including center visits and home care.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited NP program.
• Active, unrestricted California NP license, or ability to obtain prior to start.
• National certification in Family, Adult-Gerontology, or Geriatric practice preferred.
• Current DEA registration, or ability to obtain.
• Experience with geriatric or medically complex populations preferred.
• Prior experience in a PACE program or similar interdisciplinary care model preferred.

Responsibilities

• Provide primary care services to a panel of PACE participants, collaborating with supervising physician.
• Conduct comprehensive health assessments and manage acute and chronic conditions within scope of practice.
• Serve as an active member of the interdisciplinary care team (IDT) and participate in IDT meetings.
• Coordinate participant care across settings, including center-based visits and home care coordination.
• Order, interpret diagnostic testing, and prescribe medications within scope of practice and collaborative agreement.
• Maintain accurate documentation in the electronic medical record and support quality improvement initiatives.
