Mountain View Pace in Kentucky is seeking a Nurse Practitioner to assess, manage, and coordinate care for chronically ill geriatric participants, working with physicians and an interdisciplinary team to deliver safe, high-quality care. Qualifications

• Must be licensed as a Certified Nurse Practitioner in the state of practice.
• One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.
• Requires valid State driver’s license with a clean driving record.
• Successful completion of background check, physical, drug screening, TB Screening

Responsibilities

• Assess, manage, and coordinate healthcare for chronically ill geriatric participants.
• Collaborate with physicians and interdisciplinary team members to support optimal participant outcomes.
• Perform health histories, physical exams, patient education, and follow-up on diagnostic results.
• Support clinical operations, including documentation, scheduling, and charting.
• Participate in quality improvement initiatives and care planning activities.
• Serve as a clinical resource for staff, participants, and families.
