Mountain View PACE, part of the One Senior Care family, is growing across Kentucky and expanding into Ashland. We seek a Nurse Practitioner to assess, manage, and coordinate healthcare for chronically ill geriatric participants, working closely with physicians and an interdisciplinary team to ensure high-quality care.

• Licensed as a Certified Nurse Practitioner in the state of practice.
• At least 1 year of experience working with frail and elderly populations or willingness to participate in extensive training.
• Valid driver’s license with clean driving record.
• Successful completion of background check, health and safety screenings, and any healthcare program clearances.

Responsabilidades

• Assess, manage, and coordinate healthcare for chronically ill geriatric participants.
• Collaborate with physicians and interdisciplinary team to support optimal outcomes.
• Support clinical operations including documentation, scheduling, and charting.
• Participate in quality improvement initiatives and care planning activities.
• Serve as a clinical resource for staff, participants, and families.
• Contribute to 24-hour care coordination.
• Attend meetings and complete other duties as assigned.
