Job summary

Parker Jewish Institute for Health Care and Rehabilitation in New Hyde Park, NY seeks a Part-Time Nurse Practitioner for overnight shifts on weekends. You will deliver compassionate medical care to residents in subacute, long-term, rehab, and outpatient settings, reporting to Medical Director or Attending Physician.

Responsibilities include examining residents, updating care plans, educating residents and families, and documenting in EMR.

Qualifications

• Two or more years of experience in a sub-acute or long term care facility.
• Graduate of an accredited nurse practitioner program.
• Active NY NP license with prescriptive privileges.
• NYS Board Certified is required.
• Proficient in SigmaCare or another EMR (Electronic Medical Record) software.
• Prior geriatric population experience is a plus.

Responsibilities

• Examine residents and deliver quality care in subacute, long term, rehab, and outpatient settings.
• Assist in resident/patient/family education and re-evaluate and modify care plans as necessary.
• Maintain proper documentation in resident charts.
