Job summary

The Parker Jewish Institute for Healthcare and Rehabilitation in New Hyde Park, NY is hiring a Nurse Practitioner for Primary Care. This full-time role serves in the Medicine Department, reporting to Medical leadership, and delivering compassionate, quality care to residents across subacute, long-term, rehab and outpatient settings.

Responsibilities include examining residents, educating families, updating care plans, reviewing diagnostics, and supervising medical/PA students.

Qualifications

• Graduate of an accredited nurse practitioner program.
• Active NYS NP license with prescriptive privileges.
• NYS Board Certified is required.
• Experience with geriatric population preferred; 2+ years of experience in a sub-acute or long-term care facility.

Responsibilities

• Examining residents and delivering quality care in varied settings including subacute, long term, rehab and outpatient care
• Assist in resident/patient/family education
• Re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts
• Review all diagnostic testing results and make recommendations to findings
• Educate and supervise rotating medical and physician assistant students
