Job summary

The Parker Jewish Institute for Health Care and Rehabilitation in New Hyde Park, NY, is seeking a Nurse Practitioner to join our Medicine Department on a full-time basis. The NP will work with medical leadership to deliver compassionate, high-quality care to residents across subacute, long-term, rehab and outpatient settings.

Responsibilities include evaluating residents, educating patients and families, updating care plans, reviewing diagnostics, and mentoring students.

Qualifications

• Graduate of an accredited nurse practitioner program.
• Active New York NP license with prescriptive privileges.
• NYS Board Certified is required.
• Proficient in SigmaCare or another EMR software.
• Prior geriatric sub-acute/long-term care experience preferred.

• Examining residents and delivering quality care in varied settings including subacute, long term, rehab and outpatient care.
• Assist in resident/patient/family education.
• Re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts.
• Review all diagnostic testing results and make recommendations to findings.
• Educate and supervise rotating medical and physician assistant students.

Now hiring for the following shifts: 11:00am-11:00pm or 3:00pm-11:00pm The Parker Jewish Institute for Healthcare and Rehabilitation, located in New Hyde Park, NY, is seeking a Nurse Practitioner to join our team of dedicated specialists working in the Medicine Department.

In this role working alongside caring, dedicated, and experienced Medical leadership, the Nurse Practitioner will report to the Medical Director, or the supervisory Attending Physician, and is responsible for the delivery of compassionate and quality medical care of assigned residents in coordination with other members of the health care team.

Position Qualifications

• Graduate of an accredited nurse practitioner program
• Active license as an NP in New York State, with prescriptive privileges
• NYS Board Certified is required
• Proficient in SigmaCare or another EMR (Electronic Medical Record) software
• Prior experience with the geriatric population preferred; Two (2) or more years of experience in a sub-acute or long term care facility
