Job summary
Parker Jewish Institute for Health Care and Rehabilitation in New Hyde Park, NY, is seeking a Part-Time Nurse Practitioner for the overnight weekend shift (11:00pm–7:00am). You will work with a dedicated medical leadership team to deliver compassionate, quality care to residents across subacute, long-term, rehab, and outpatient settings.
Responsibilities include exams, care plan adjustments, and patient education, with documentation in resident charts.
Qualifications
- Two+ years of experience in sub-acute or long-term care.
- Graduate of an accredited nurse practitioner program.
- Active NP license in New York State with prescriptive privileges.
- New York State Board Certified is required.
- Proficient in SigmaCare or another EMR system.
- Prior geriatric population experience is a plus.
Responsibilities
- Examine residents and deliver quality care in subacute, long-term, rehab, and outpatient settings.
- Assist in resident/patient/family education and modify care plans as necessary.
- Maintain proper documentation in resident charts and coordination with the health care team.
