Job summary

Parker Jewish Institute for Health Care and Rehabilitation in New Hyde Park, NY is seeking a Part-Time Nurse Practitioner for overnight weekend coverage (11:00pm–7:00am) to join our Medical Department. You will examine residents, deliver quality care, and coordinate with the health care team, reporting to Medical Director or attending physician, and document care plans.

Applicants should be NYS-licensed NP with prescriptive privileges, board certification, and experience in subacute/long-term

Qualifications

• Two or more years of experience in a sub-acute or long term care facility.
• Graduate of an accredited nurse practitioner program.
• Active NYS NP license with prescriptive privileges.
• NY State Board Certified is required.
• Proficient in SigmaCare or another EMR software.
• Prior geriatric population experience is a plus.

Responsibilities

• Examine residents and deliver quality care in subacute, long term, rehab, and outpatient settings.
• Assist in resident/patient/family education and re-evaluate care plans.
• Maintain documentation in resident charts and coordinate with care team.
