Job summary

CareMed is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. The NP will manage acute changes in condition, plan advanced and chronic care, and collaborate with primary care physicians to optimize resident health.

Responsibilities include prescribing medications per Nebraska law, ordering and following up on treatments, and coordinating with nursing home staff to adjust care plans as conditions change.

Qualifications

• Current, unrestricted NP license in Nebraska.
• Board certification in family, adult, or geriatric care.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus.

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with Nebraska law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed basis to discuss changes in condition.
• Collaborate with primary care physician.
