Essen Health Care in New York seeks motivated Nurse Practitioners and Physician Assistants to join our team serving multiple nursing homes across NYC. This is an excellent opportunity for new grads to start in a supportive, patient-focused environment.

The NP/PA will perform routine primary care, chronic disease management, and coordinate with facility staff and supervising physicians. Training is required at our New York locations before transitioning to Bergen County, NJ; commuting to NY

Qualifications

• Master's degree in Nurse Practitioner (Family or Adult Gerontology).
• Current NY NP license is required.
• National board certification required (AANP/ANCC/ONCC).
• Bilingual Spanish is a plus.

Responsibilities

• Provide routine primary care and follow-up visits in long-term care facilities.
• Collaborate with supervising physicians and care teams.
• Document comprehensive patient information and care plans.
