Qualifications

• Master's degree required in NP program (Family or Adult Geriatrics).
• Current New York NP license.
• National board certifications required (AANP/ANCC/ONCC).

• EMR/EHR experience a plus.

Responsibilities

• Examine, diagnose and treat patients.
• Provide advanced primary and/or specialty care and create care plans.
• Order or perform tests and imaging as needed.
• Vaccinate and advise on prevention.
• Refer to subspecialists within network.
• Document patient information accurately.
