Job summary

Pine Park Health, Inc. is hiring a full-time Nurse Practitioner to deliver primary care to older adults in senior living communities across the East Bay, including Hayward. You will manage a smaller patient panel, develop personalized care plans, and collaborate with RNs, MAs, and care navigators for coordinated care.

Qualifications

• Active, unrestricted California NP license with Furnishing Number.
• Active, unrestricted DEA registration (II–V).
• Willingness to travel between partner communities in the East Bay.
• Ability to work in a team with RNs, MAs and care navigators.
• Comfort with tech-enabled care tools and documentation.

Responsibilities

• Provide comprehensive primary care to older adults in senior living communities.
• Develop and manage individualized care plans aligned with patient goals.
• Deliver CCM visits and complete documentation monthly.
• Collaborate with care teams to coordinate follow-up and continuity of care.
• Contribute to improving clinical workflows with medical leadership.
