Pine Park Health, Inc. is hiring a full-time Nurse Practitioner to provide primary care to older adults in senior living communities across Hayward and the East Bay.

You will manage a smaller panel, develop personalized care plans, and collaborate with RNs, MAs, and care navigators to deliver proactive, top-of-license care onsite with travel between communities. You’ll work from home on Fridays, with a strong emphasis on documentation, care coordination, and professional development, supported

Qualifications

• Active, unrestricted CA NP license with Furnishing Number.
• Interest in caring for medically complex older adults and delivering coordinated, preventive primary care.
• Willingness to provide onsite care across the East Bay (Hayward), including driving between partner communities.
• Adaptability and comfort in a growing care model.
• Strong collaborator who values input from every member of the clinical team.

Responsabilités

• Provide comprehensive primary care to older adults in senior living communities.
• Develop and manage individualized care plans.
• Deliver monthly CCM visits and complete documentation.
• Collaborate with RNs, MAs, care navigators, and staff to coordinate care.
• Lead regular clinical huddles to review patient needs and follow-up.
• Partner with community leadership to implement care plans in living environments.
• Collaborate with Medical Director to improve workflows and policies.
