Physician Solutions Group in York, PA, is seeking a full-time CRNP to join our growing nurse practitioner team caring for chronically ill geriatric members. This role emphasizes independence and collaborative care with physicians and interdisciplinary teams.

NO WEEKENDS, NO EVENINGS, AND NO HOLIDAYS, offering a stable schedule and a patient-centered approach to improve quality of life at home and in community settings.

Qualifications

• Licensed CRNP with at least one year in geriatrics or chronically ill populations.
• Experience delivering in-home or community-based care preferred.
• Strong assessment, ordering, and documentation skills.

Responsabilités

• Perform health histories and in-depth medical exams for new members to determine care plans.
• Initiate, evaluate and review medical orders and seek improvement opportunities.
• Carry out procedures and document care and recommendations.
• Order, interpret and follow up on lab tests and results.
• Coordinate consultations with physicians, therapists, social workers and other specialists.
• Educate staff, members, caregivers and families to maximize independence at home.
• Complete ongoing assessments to update Plans of Care based on feedback and evaluations.
