Creative Healthcare Solutions, LLC seeks a Nurse Practitioner to serve geriatric patients in a skilled nursing facility in Bowling Green, KY. You will provide primary care with a low patient volume, focusing on comprehensive assessment and management of chronic and acute conditions.

Responsibilities include directing medical care, ordering tests, and collaborating with an interdisciplinary team. On-call duties are limited, schedule is Monday–Friday with some flexibility, and full benefits are

Qualifications

• KY NP license required.
• DEA registration required.
• 1+ year advanced practice experience in geriatrics or related settings.
• 1+ year experience in facilities, home care, geriatrics, etc.

Responsibilities

• Plan, direct, and evaluate medical care for geriatric patients in a skilled nursing facility.
• Examine and treat chronic and urgent illnesses.
• Order and interpret laboratory and diagnostic tests.
• Collaborate with caregivers and interdisciplinary care team.
• Be on-call with low volume nights (no more than 3 per month) and schedule flexibility.
