Novenza Health in Kokomo, IN is seeking dedicated Nurse Practitioners to join our Post-Acute LTC team. You will assess, diagnose and manage patient care in nursing home and assisted living settings, collaborating with physicians to improve residents' quality of life.

Qualifications

• Current Nurse Practitioner license in Indiana with relevant certifications in adult, geriatrics or family practice.
• Experience in long-term care settings preferred but not required.
• Proficiency in EMR systems Ethizo, GEHRIMED, Matrix, PCC.
• Knowledge of ICD-9/ICD-10 coding.
• Background in critical care or hospital medicine is a plus; trauma center experience desirable.
• Strong communication skills and ability to collaborate within a multidisciplinary team.

Responsabilités

• Conduct comprehensive physical examinations and health assessments for residents in long-term care settings.
• Develop and implement individualized care plans based on patient needs.
• Manage acute pain, behavioral health issues, and chronic conditions.
• Collaborate with physicians and other healthcare professionals to coordinate patient care.
• Maintain accurate and timely medical documentation in HIPAA-compliant EMR/EHR systems.
• Educate patients and families on health management and disease prevention.
• Participate in staff/facility meetings to improve patient outcomes and clinical practices.
• Ensure adherence to infection control protocols and best practices.
