Novenza-Health is seeking a dedicated Nurse Practitioner to join our Post-Acute Long-Term Care team in Indiana. The role emphasizes autonomy while maintaining close collaboration with physicians to deliver high-quality geriatric care in nursing homes and assisted living facilities.

Qualifications

• Current Nurse Practitioner license in the state of practice.
• Experience in long-term care settings is preferred but not required.
• Proficiency with EMR/EHR systems (Ethizo, GEHRIMED, Matrix, PCC).
• Knowledge of ICD-9/ICD-10 coding.

Responsabilités

• Conduct comprehensive physical examinations and health assessments for patients in long-term care settings.
• Develop and implement individualized care plans based on patient needs and preferences.
• Manage acute pain, behavioral health issues, and chronic conditions with effective treatment strategies.
• Collaborate with physicians and other healthcare professionals to coordinate patient care.
• Maintain accurate medical documentation in HIPAA-compliant EMR/EHR systems.
• Provide education to patients and families regarding health management and wellness strategies.
• Participate in staff/facility meetings to improve patient outcomes and clinical practices.
• Ensure adherence to infection control protocols and best practices in patient care.
