Novenza Health in Merrillville, Indiana seeks a dedicated Nurse Practitioner to join our Post-Acute LTC team. You will assess, diagnose, and manage care for residents in nursing homes and assisted living settings, working closely with physicians to maintain high-quality patient outcomes.

Qualifications

• Current Nurse Practitioner license in Indiana with certifications in adult, geriatrics or family practice.
• Experience in long-term care settings such as nursing homes or assisted living facilities is preferred.
• Proficiency in EMR systems (Ethizo, GEHRIMED, Matrix, PCC) for medical documentation.
• Knowledge of ICD-9/ICD-10 coding.
• Background in critical care or hospital medicine is a plus; strong communication skills are essential.

Responsabilités

• Conduct comprehensive physical examinations and health assessments for patients in long-term care settings.
• Develop and implement individualized care plans based on patient needs and preferences.
• Manage acute pain, behavioral health issues, and chronic conditions with effective treatment strategies.
• Collaborate with physicians and other healthcare professionals to coordinate patient care.
• Maintain accurate medical documentation in compliance with HIPAA using EMR/EHR systems.
• Provide education to patients and families regarding health management and wellness strategies.
• Participate in staff/facility meetings to improve patient outcomes and advance clinical practices.
• Ensure adherence to infection control protocols and best practices in patient care.
