Job summary

Element Care is seeking a Registered Nurse in Massachusetts to join the PACE Interdisciplinary Team. This full-time role runs Monday through Friday, 8am–4pm, with a focus on planning, implementing and evaluating care plans to keep frail elders safely in the community.

You will perform assessments, coordinate medications, and collaborate with physicians and caregivers to deliver high-quality nursing care and education. Travel locally as needed in a personal vehicle.

Qualifications

• Current MA RN license.
• Minimum 2 years geriatric experience.
• Proficient with Microsoft Office.
• Willingness to travel locally in a personal vehicle.

Responsibilities

• Provide direct care to participants; complete assessments and revise care plans.
• Collect medical and social information to support decisions for safety and well-being.
• Monitor clinic care and service delivery to meet QA standards.
• Coordinate with the interdisciplinary team to develop comprehensive care plans.
• Ensure tests and diagnostics ordered by physician are carried out.
