You will perform routine rounds, manage chronic disease, and coordinate care with interdisciplinary teams to improve outcomes and patient experience.

Experience in geriatrics or family/internal medicine is preferred. Strong communication and collaboration skills are essential to build trust with residents, families, and facility staff.

Qualifications

• Current NP or PA licensure in the state of practice.
• Experience with geriatrics, family medicine, or internal medicine preferred.
• Strong patient-centered service and professional communication.
