Job summary

CHS Recruiting is seeking a Nurse Practitioner – Geriatric Care in Norwalk, Ohio. This full-time role serves geriatric patients in long-term care facilities with a dedicated panel and 40–60 visits per week, supported by a mobile-based EHR with AI features.

The schedule is Monday through Friday with no evenings, weekends, or major on-call. New graduates are welcome, and a thorough training program is provided to ensure smooth transition into geriatric primary care.

Qualifications

• Ohio NP license
• DEA certification
• New grads with backgrounds in home health, LTC, ER, etc. are encouraged to apply
• No experience required

Responsibilities

• conduct health assessments
• develop personalized care plans
• evaluate patient conditions and responses to treatment
• adjust treatment plans as necessary
• perform minor procedures and wound care (as needed)
• educate patients and families on health management, disease prevention, etc.
• provide guidance on lifestyle modifications and self-care practices
• work with physicians, nurses, and other professionals to coordinate care
