Job summary

Qualifications

• Board Certification is required.
• Active DEA license is required.
• Adult, Family, or Geriatric clinical background.
• Previous long-term care/skilled nursing experience.
• Ability to manage patients with complex medical conditions.
• Strong communication and teamwork skills.
• Psychiatric/behavioral health experience is a plus.

Responsibilities

• Provide primary and preventative care to long-term care residents.
• Evaluate and treat acute changes in condition.
• Manage chronic diseases and advanced illness.
• Develop and participate in care planning and advanced care planning.
• Prescribe medications, including controlled substances in accordance with Ohio law.
• Order and follow up on labs, imaging, treatments, and diagnostic testing.
• Communicate with nursing facility staff regarding changes in condition and care plans.
• Collaborate with physicians and an interdisciplinary care team.
• Perform medication management, preventive care, and wellness visits.
• Utilize an EMR for clinical documentation.
• Participate in wound care and minor procedures as appropriate.
