Job summary

CommuniCare Health in Cincinnati, OH is seeking a Nurse Practitioner (Adult and/or Gerontology) to provide high-quality primary care to seniors and residents in skilled nursing, assisted living, and via telehealth. This full-time NP will work first shift, Monday–Friday, with no on-call, weekends, or holiday coverage.

Qualifications

• Master of Science in Nursing or higher.
• National certification (AANP/ANCC) active, unrestricted license.
• Active DEA license and US work authorization.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Develop treatment plans and communicate them to residents and providers.
• Order and interpret labs and adjust treatments as needed.
• Maintain compliant documentation within EMR software.
• Collaborate with PCPs and care teams to improve outcomes.
