Qualifications

• Active Nurse Practitioner license in the state of practice.
• Board Certification as NP or equivalent.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and relationship-building abilities.
• Ability to work independently while collaborating with facility teams.

Responsabilités

• Serve as the primary care provider for assigned residents.
• Perform initial assessments and routine follow-up visits.
• Manage chronic and acute medical conditions.
• Round daily to maintain a visible clinical presence.
• Collaborate with nursing staff, therapists, social workers, and administration.
• Respond to resident condition changes with timely interventions.
• Participate in care plan meetings and interdisciplinary discussions.
• Communicate care plans and medical concerns with residents and families.
• Help reduce avoidable hospitalizations through proactive management.
• Maintain compliant medical documentation.
• Support facility quality improvement and regulatory readiness.
