TruCare Management is seeking a full-time Nurse Practitioner who wants to become an integral part of a skilled nursing facility and make a real difference in the lives of residents every day.

Required Qualifications

• Active Nurse Practitioner license in the state of practice.
• Board Certified Nurse Practitioner (FNP, AGNP, ANP, or equivalent).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and relationship-building abilities.
• Ability to work independently while collaborating effectively with facility teams.

Preferred Qualifications

• Experience in Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Assisted Living, or Geriatrics.
• Experience caring for medically complex patients.
• Familiarity with PointClickCare or similar EHR systems.
