Qualifications
- Licensed RN with current state licensure.
- Graduate of an accredited nursing program.
- Certified Nurse Practitioner with applicable certification.
- At least 1 year of experience working with elderly populations.
- Training or related experience in the role.
Responsibilities
- Performing physical assessments and developing individualized care plans.
- Evaluating participant complaints and administering treatments.
- Conducting semi-annual reassessments and in-person evaluations for new enrollees.
- Collaborating with an Interdisciplinary Team to ensure coordinated care.
- Providing education and guidance to participants and caregivers.
- Overseeing residents in skilled nursing facilities and coordinating with primary care providers.
- Participating in discharge planning and managing emergent and acute needs.
