Job summary

TruCare Management is seeking a full-time Nurse Practitioner to provide primary care within skilled nursing facilities, serving as a trusted clinical presence throughout the buildings.

The ideal candidate will round daily, manage chronic and acute conditions, and collaborate closely with nursing staff, therapists, social workers, and administrators to improve resident outcomes and reduce avoidable hospitalizations.

Qualifications

• Active NP license in state of practice (required).
• Board Certified NP (FNP, AGNP, ANP, or equivalent).
• Strong clinical assessment and critical thinking.
• Excellent communication and relationship-building abilities.
• Ability to work independently while collaborating with facility teams.

Responsibilities

• Serve as the primary care provider for assigned residents.
• Perform initial assessments and routine follow-up visits.
• Manage chronic and acute medical conditions.
• Round daily and maintain a visible clinical presence.
• Collaborate with nursing staff, therapists, social workers, and administration.
• Respond to resident condition changes and provide timely interventions.
• Participate in care plan meetings and interdisciplinary discussions.
• Communicate with residents and families regarding care plans and medical concerns.
• Help reduce avoidable hospitalizations through proactive clinical management.
• Maintain accurate and compliant medical documentation.
• Support facility quality improvement and regulatory readiness initiatives.
