Ultra Health Providers seeks a Part-time Nurse Practitioner (NP) to deliver primary, preventive, and urgent care to residents in skilled nursing and assisted living communities in Tennessee. You will collaborate with a clinical team to optimize resident outcomes and support nursing staff and administrators.

The ideal candidate holds an active TN NP license with national board certification (AANP/ANCC) and 1–2 years of geriatric or primary care experience, demonstrating strong clinical judgment

Qualificações

• Active and unrestricted Nurse Practitioner license in Tennessee.
• Current national board certification (AANP or ANCC).
• Minimum 1-2 years of experience in primary care, geriatric, or long-term care settings preferred.
• Strong clinical judgment and communication skills.
• Commitment to compassionate, patient-centered care.

Responsabilidades

• Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings.
• Conduct admission assessments, physical exams, medication reviews, and depression screenings.
• Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care.
• Counsel patients and family members on treatment options and changes in health status.
• Manage clinical documentation accurately and in compliance with facility and state standards.
