Job summary

Ultra Health Providers in Tennessee is seeking a compassionate Nurse Practitioner to deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living communities. You will work a part-time, Monday–Friday schedule and collaborate with nursing staff and administrators to ensure excellent patient outcomes.

The ideal candidate holds an active Tennessee NP license, board certification (AANP or ANCC), and 1–2 years in primary care, geriatric, or long-term care settings.

Qualifications

• Active and unrestricted Nurse Practitioner license in Tennessee.
• Current national board certification (AANP or ANCC).
• Strong clinical judgment and communication skills.

Responsibilities

• Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings
• Conduct admission assessments, physical exams, medication reviews, and depression screenings
• Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care
• Counsel patients and family members on treatment options and changes in health status
• Manage clinical documentation accurately and in compliance with facility and state standards

Role Overview

As a Nurse Practitioner with Ultra Health Providers, you will serve as a trusted clinician for residents in your assigned facility. You’ll manage acute and chronic conditions, provide preventive care, and partner closely with nursing staff and administrators to ensure exceptional patient outcomes.
