Job summary

Ultra Health Providers is seeking a Nurse Practitioner to provide high-quality primary and preventive care to residents in skilled nursing and assisted living communities in Tennessee. This part-time role offers a Monday–Friday schedule with a supportive clinical team and mission-driven environment.

You will manage acute and chronic conditions, coordinate with nursing staff and physicians, and deliver compassionate, patient-centered care to an aging population.

Qualifications

• Active and unrestricted Nurse Practitioner license in Tennessee.
• Current national board certification (AANP or ANCC).
• Minimum 1-2 years of experience in primary care, geriatrics, or long-term care settings preferred.
• Strong clinical judgment and communication skills.
• Commitment to compassionate, patient-centered care.

Responsibilities

• Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings
• Conduct admission assessments, physical exams, medication reviews, and depression screenings
• Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care
• Counsel patients and family members on treatment options and changes in health status
• Manage clinical documentation accurately and in compliance with facility and state standards
