Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably geriatrics. This role mainly serves residents in nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You will collaborate with a team of providers and RN case managers, exercising independent judgement to improve outcomes and prevent unnecessary hospital visits, while enjoying a competitive

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Microsoft Word, Outlook and Excel proficiency.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages conditions.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to treatment and modify plans accordingly.
• Collaborate with RN case managers as part of an integrated care team.
