Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric and primary care experience to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers to deliver coordinated health care within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You will exercise independent judgement to treat acute and chronic conditions, consulting with a supervising physician as appropriate.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages their conditions.
• Ensure compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to their treatment and modify plans accordingly.
• Collaborate with RN case managers as part of an integrated care team.
