Job summary

TruHealth, part of American Health Partners, seeks an advanced practice provider to deliver primary care in nursing homes and assisted living communities. You will collaborate with a care team to improve residents' health outcomes and quality of life.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a proactive plan of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor treatment effectiveness and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
