Job summary

Carle Health in Champaign-Urbana, IL seeks an advanced practice provider to join our geriatrics care team. You will work with geriatricians, APPs, nurses and staff to serve nursing homes, assisted living and memory care residents, with a M-F schedule and rotating weekend/overnight call.

The role emphasizes health assessments, diagnostic testing, and following a mutual treatment plan while documenting care and communicating with families and referral agencies.

Qualifications

• Master's degree in Nursing or Physician Assistant.
• Background working with geriatric patients is ideal; training new grads is possible.

Responsibilities

• Collaborate with geriatricians, APPs, nurses and staff to care for seniors in nursing facilities and memory care.
• Perform health assessments, order labs and diagnostics, and follow treatment plans.
• Educate families and coordinate with referral agencies as needed.
• Support precepting and participate in quality improvement projects.
